Waiver of Liability and Release Form
In consideration of my child’s participation in all activities of organized soccer, sport programs or other related events, conducted at Victoria Sport Park, located at 15555 W. 52nd Ave. in Golden, CO.
I agree and acknowledge that:
In consideration of our/my child being allowed to participate on behalf of Victoria Sports Complex athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that: Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and, I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my participation. I/We willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I/We observe and any unusual or significant hazard during my presence or participation, I/We will remove our/my child from participation and bring such to the attention of the nearest official immediately. I/We, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Victoria Soccer Complex., their officers, officials, coaches, members, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“releasees”), while participating in activities of any kind whether sponsored by or under the supervision of Victoria Sports Complex and with respect to any and all illness, disability, death, or loss or damage to person or property, whether arising from the negligence of releasees or otherwise, to the fullest extent permitted by State law.
We agree and comply to wear protective gear such as shin guards, appropriate shoes, or other protective equipment deemed necessary for their health and safety. I have the right to inspect the facility and equipment prior to their participation. If I believe to find anything unsafe, I will advise the coach or supervisor and may refuse to participate. We, the Parents, give permission for emergency medical treatment of our child for illness or accident if we can not be contacted.
Victoria Sports Club reserves the right to use, for any purpose related to club activities and without compensation, any photographs, videos, audio tapes, or other recordings that are made on the premises or at any Victoria Sports Club related function or event.
I have completely read this document and my participation assumes consent to all the above. I release, waive liability.
Park Rules
Victoria Sports Club is a members club and home to Soccer fans, individual players and teams, throughout the Denver Metro Area and the world.
Victoria Sports Club is for everyone and prohibits discrimination on the basis of race, color, religion, gender or national origin.
Victoria Sports Club rules and regulations apply to all members as well as their guests.
The use of turf and grass fields are available to members by reservation or prior approval only and may require additional fees. Guest players are only allowed on the fields in the presence of member(s) Proof of membership must be provided with a club card if asked by the onsite attendant.
Usage of the field is for play only. No spectators are allowed on the field.
1. All members attending or using the facilities, if asked by the onsite attendant must provide their membership card.
2. Turf and grass fields are for members only. Available by reservation or prior approval only and may require additional fees. See a Membership Advisor for guest policies and fees.
3. Members may not conduct personal training or utilize outside personal trainers.
4. All members are required to wear proper attire when using the Victoria Sports Club facility. Attire containing inappropriate words, phrases, or graphics is prohibited.
5. The use of profanity and/or loud and intimidating behavior is prohibited.
6. Proper etiquette is required in all areas of the sports club. Management has the right to cancel any membership for unacceptable behavior or conduct which in the opinion of management is prejudicial to the welfare, good order and character of the Victoria Sports Club. The use of profanity and/or loud and intimidating behavior is prohibited.
Noise restrictions:
Since the facility is close to residential neighbors, all music-playing and other portable devices must be set at reasonable levels.
Victoria Sports Club reserves the right to use, for any purpose whatsoever and without compensation, any photographs, videos, audio tapes, or other recordings that are made on the premises or at any Victoria Sports Club related function or event.
7. Food and beverages in open containers are restricted on the playing field at all times. No alcoholic beverages are allowed to be brought on to the premises at any time.
8. Service animals are welcomed and must be leashed at all times. Documentation may be required. No pets allowed on the fields at any time.
9. Smoking, including electronic cigarettes, is prohibited within all facilities.
10. Children under the age of 14, who have not completed Youth Experience, may not be left unattended in any area of the club with the exception of a staff led program.
11. As of October 1st, 2022 dogs (pets) will no longer be allowed inside the gated facility of Victory Sports Park.
Children under the age of 7 must be accompanied by a parent/guardian at all times and not left unattended in any area of the club.
